The series concludes in Bedlam, a well-known London mental health hospital that was notorious for its harsh and neglectful treatment of patients. Tom has descended into madness. Stripped of status and dignity, he is observed by fashionable visitors who treat the asylum as entertainment. Sarah Young mourns beside him, providing a final contrast between steadfast compassion and moral ruin. The narrative arc culminates in total collapse: wealth, reputation and reason are lost, presenting a cautionary allegory of moral failure in eighteenth-century England urban life.

This final verse tells us that a mind obsessed by greed and vanity can become confused and ultimately ruined. It says that a parent’s poor choices can lead their child into suffering and ultimately destruction:

"Madness! thou Chaos of the brain; What art, that pleasure giv'st and pain? Tyranny of Fancy's reign! Mechanic Fancy! that can build Vast labyrinths and mazes wild, With rule disjointed, shapeless measure, Fill'd with Horror, fill'd with Pleasure! Shapes of Horror, that wou'd even Cast doubt of mercy upon heaven! Shapes of Pleasure, that but seen Wou'd split the shaking sides of Spleen! Of vanity of age! here see The stamp of heav'n effac'd by thee! The headstrong course of youth thus run, What comfort from this darling son? His rattling chains with terror hear; Behold death grappling with despair; See him by thee to ruin sold, And curse thyself, and curse thy gold".
